Work with clinical colleagues and subject matter experts, including those with lived experience, to learn approaches that build capability.

APPLY WHAT WORKS
Use practical resources and strategies that support the emotional health of patients with chronic conditions and their families. Physicians collecting simple data and testing changes are eligible for MOC Part 4 credit.

Roadmap Collaborative
Launching in spring 2023, the Roadmap Collaborative is a year-long improvement collaborative that includes 38 teams from 17 hospitals and aims to ensure that participating clinicians assess the emotional health of children and youth with chronic conditions and their families at every encounter.
The Roadmap Collaborative 2023 is currently closed for recruitment.

Roadmap College
Roadmap College was a six-month webinar series that began in May 2022. During the series, Roadmap partnered with 16 pediatric cardiology teams to integrate tested Roadmap tools and strategies into practice to support the emotional health of patients and families.
