Follow these three steps to prepare your team for Roadmap College. Reference the documents below for additional details.
1) Determine a team aim (template provided in the reporting guidance document)
2) Complete the Roadmap Readiness Checklist
3) Establish a baseline for one measure, documentation of assessing patient and/or family health, by reviewing 10 patient charts from providers seen by your Roadmap College team over the last month
